There are two options for applying for membership.
1) An online form which you fill out and click the submit button.
2) Click and download a physical form which you fill out, and
mail in with a cheque, or bring to a physical meeting.

Completed forms can be emailed to email@example.com and your fees paid by eTransfer. OR Completed forms can be mailed, or brought to a monthly meeting, along with a cheque for your fees.
